An increasing number of community-dwelling persons have impairments that prevent them from leaving their homes without personal assistance or leaving their homes at all. The homebound or semi-homebound, persons most often have three or more chronic conditions limiting them to home. These persons most often have higher rates of medical emergency use than the non-homebound.

It is New Era Wellness’s goal to become the primary care provider and manage the chronic issues and concerns of the semi to homebound client. The objective is to lower emergency room visits, decrease hospital admission, and promote longer community-based living for as long as life allows.
